  • Photo ID (such as a valid driver’s license)
  • Insurance card (for those with dental insurance) 
  • Questions, concerns, and hopes for your smile

Prior to Your Visit

Please fill out the online New Patient Forms prior to your visit.
*if you are unable to fill out the paperwork online, please arrive 20 minutes before your scheduled appointment to complete the paperwork.

Finances and Insurance

Our experienced team will help you navigate the insurance maze. We also offer a wide variety of convenient payment options including financing through CareCredit.

No Insurance? No problem.

For patients without dental insurance we offer a Dental Savings Plan that allows you to receive two exams, two cleanings, necessary x-rays, and fluoride treatments for a low annual fee. The plan also includes a 15% discount on all other dental treatments in our office during the course of your enrollment.
